1ST TIME TRAVELERS NO CALL/NO WEEKENDS REQUIRED CERTS: BLS MUST
HAVE 2 YEARS ONCOLOGY EXPERIENCE. MUST HAVE SOME CASE MANAGEMENT
EXPERIENCE PEDS EXP PREFERRED Patient Care manager role supporting
the Oncology patients. Will be responsible for working in a
multidisciplinary team to develop a road map for this patient
population. Accountable for assessment of patients, implementation
of physician orders, treatment plans, medical procedures, record
keeping, as well as monitoring and evaluating patient care
processes and outcomes. Coordinate activities with other services
and disciplines. Must have an Oncology background Must have the
following skills: Responsible for road map - will be customized per
patient Nursing/ care management evaluation cross continuum
inpatient and outpatient clinic coordinating follow up appointments
ordering and follow up of labs/appointments/scans Referrals
authorization DME ordering/coordination Care Coordination
Transition planning Needs Assessment Experience with Oncology
