A company is looking for a Population Health Coordinator.
Key Responsibilities

Manage data and registries to support care coordination and quality improvement initiatives
Coordinate care for complex members and assist in training staff on the Collaborative Care Model
Provide direct member care, including outreach, assessments, and education on health and recovery options

Required Qualifications, Training, and Education

For Medical Focus: RN or LPN (Maine only) with SAMHSA training and 5 years of experience in addiction medicine or behavioral health
For Mental Health Focus: Licensed clinical social worker with 5 years of experience in mental health or integrated behavioral health
For Social Care Focus: Current social worker or certified peer/community health worker with 5 years of experience in case management or peer services
Access to reliable internet and telephone services for remote roles
Must meet pre-employment requirements including background screening and possible health screenings
