Key Responsibilities
Perform outreach to members to assist with care plan next steps and
connect them to health plan and community resources
Serve as the front-line support for member and provider inquiries
regarding care plan procedures and protocols
Document and maintain non-clinical member records in compliance
with state and regulatory requirements
Required Qualifications
High School diploma or GED
1 - 2 years of related experience
