Job Description:
A company is looking for an RN Case Manager.
Key Responsibilities
Engage with patients to assess and prioritize their clinical
support needs
Coordinate care with healthcare teams and provide education for
successful transitions back to home settings
Implement interventions to improve health outcomes and assist
patients in navigating the healthcare system
Required Qualifications
Bachelor's Degree in Nursing
5 years of experience as an RN or RN Care Manager
Post Acute Care and Transitions of Care experience strongly
preferred
Unencumbered RN license; compact nursing license preferred
Accredited Case Manager (ACM) preferred
